French Camp, CA — July 20, 2010, the Health Plan of San Joaquin (HPSJ) will award up to $150,000.00 in grants to San Joaquin County nonprofit organizations through its Community Wellness Funding Program.
Multiple grants of up to $25,000 will be awarded in support of projects targeting the following issues: access to health care, ethnic and cultural disparities in health, or child fitness and nutrition.
Agencies may initiate the application process through a brief letter of intent due no later than 4:00 p.m. on August 12, 2010, at the HPSJ. The program guidelines and letter of intent form are available upon request from Health Plan of San Joaquin by contacting Luisa Smith at lsmith@hpsj.com or (209) 461-2312.
Since 2007, the Community Wellness Fund was established and agencies such as, the Boys & Girls Club of Manteca – “Healthy Habits – Triple Play”; Hilliard Chapel AME Zion Church – “Mommy & Me Program”; The Salvation Army of Lodi – “Preventive Health Care Program”; St. Mary’s Interfaith Community Services – “Dental Outreach Program for Homeless Children and Families”; and the Tracy Free Clinic – “Free Health Care Access Project” have provided services to the community.
